The Armenian Association of Social Workers (AASW), within the framework of the project “RISE – Armenia: Rehabilitation, Social Inclusion and Empowerment of Displaced and Vulnerable Population,” announces a call for applications for the formation of the professional team of the Ashtarak Multifunctional Social Care Service.

Position: Care Specialist / Family Mentor (Ashtarak Multifunctional Social Care Service)
Project Title: “RISE – Armenia: Rehabilitation, Social Inclusion and Empowerment of Displaced and Vulnerable Population.”
Project Objective: To improve the social protection system in Armenia for vulnerable populations, including those displaced from Nagorno-Karabakh, through strengthening the capacities of social workers and ensuring access to new social services, including patronage, social care, and socio-rehabilitation services.
Project Duration: November 2024 – November 2027

1. Scope and Purpose
To provide practical, hands-on support and mentoring to vulnerable families within mobile (home-based) services, focusing on strengthening parenting skills, household management, daily living skills, and family resilience, in accordance with the Individual Social Plan developed by the Social Worker.
2. Duties and Responsibilities
2.1 Practical Support and Skills Development (Home-Based Services)
- Conduct regular home visits according to the Individual Intervention Plan.
- Demonstrate and teach basic household management skills (cleaning, organizing living space, maintaining hygiene).
- Teach practical cooking skills, focusing on affordable and healthy meals.
- Support families in developing budgeting and basic resource management skills.
- Assist parents in organizing daily routines and time management.
- Provide practical guidance on childcare, including hygiene and basic care for infants and children.
- Support families in improving school attendance and engagement of children.
All activities must be carried out with the family, not instead of them, aiming to strengthen independence and self-reliance.
2.2 Provision of Nursing Interventions
- Monitor medication intake and its impact on the beneficiary’s health condition.
- Provide first aid when necessary.
- Perform wound care and dressing changes in accordance with medical instructions.
- Arrange necessary laboratory tests or medical interventions based on physician recommendations.
- Accompany beneficiaries to medical institutions when required.
- Maintain regular communication with the beneficiary’s family or attending physician.
2.3 Health Counseling and Preventive Work
- Provide individual counseling on health maintenance.
- Promote healthy lifestyle practices and hygiene standards.
- Inform beneficiaries about measures to prevent deterioration of their health condition.
- Multidisciplinary Cooperation
- Collaborate with the social worker, psychologist, family mentor, and other team members.
- Participate in case discussions and multidisciplinary meetings.
- Provide health-related observations for individual intervention planning.
2.4 Documentation and Reporting
- Maintain accurate documentation of home visits and nursing interventions provided.
- Ensure confidentiality of beneficiaries’ health data.
- Submit periodic reports to the Service Supervisor.
- Participate in monitoring and evaluation processes.
2.5 Other Responsibilities
- Participate in professional capacity building activities and team sessions.
- Support donor visits and evaluation processes when required.
- Perform other duties within the scope of the position.
3. Minimum Qualification Requirements
- Relevant nursing education.
- Valid nursing license.
- At least 2 years of professional experience.
- Experience in community-based or home-visit services is an advantage.
- Strong communication skills.
- Computer literacy (MS Office and experience with online platforms).
